Description:

The main objective of the project is to propose measures for maintaining persons who require care (persons 50+ dependent on care provided by another person, including the elderly) in their home environment based on the support of carers and families. To this end, a completely new services model will be created and pilot tested and a social-health community centre will be established at the municipal level (municipality with extended powers) to which a new social-health care coordinator will appointed and who will be trained to work with the target group. The output of the project will consist of a methodology that sets out a definition of the new position and the tools, methods and assistance approaches to be applied including operational models. With respect to the long term, the project aims both to connect and create a network of social and health services and to enable financial savings with concern to investment in new capacity (especially regarding residential facilities). The aims of the project are fully in line with the National Strategy for the Development of Social Services for 2016–2025.
